Observation

The provider self-reported that a child had sunscreen applied without parent permission. This was confirmed by interviews with child care staff. Staff indicated there was a lapse between the child being enrolled and the permission log being updated. Once the error was discovered, staff responded by notifying the parent and the department. In response to the incident, the permissions paperwork for enrolled children was reviewed and the log was updated. Staff also stated that since the incident the permission log is updated with each new child enrolled and staff are diligent about reviewing the paperwork frequently to ensure sunscreen is only applied to those with parent permission.
